Salary:
$60,000 - $80,000 per hourA bit about us:
Nurse Coordinator provides comprehensive nursing care services utilizing various skills including assessment, planning, intervention, and evaluationAdditionally, handles all administrative duties to include reviewing intake packets, assist with UA's, appointments, staff clients, and update treatment team on clients status.
Nurse Coordinator Purpose:
Responsible will be responsible for day-to-day Responsible will be responsible for day-to-day administrative support and client services for NSR programs and will be the liaison between KSC and NSR, occasional travel to other sites required, mileage reimbursedWe have locations in Chaska, Eden Prairie, & Otsego.

Responsibilities:
1Create and update charts in Practice Fusion (PF) for new intakesaEnsure the packets are completed and signedbReview the intake schedule and determine needs of the client2Help clients prepare for appointments in designated areasaProvide a schedule for NSR staff a day priorbCollect vitals and update med listcLog onto doxy for teleahealh appointments and notify the provider3Consistent communication with treatment team, admissions, and Kai Shinafacilitation of referrals to substance use disorder/mental health servicas as indicated by a client's medical provider, comprehensive assessment, or treatment planbassist with treatment coordination6Assist with med auditsaReceives info from nursing/staff a list of med refillsbVerifies in PF the last refills and if they are neededcCalls pharmacy if it has been received by facilitydMessages providers in PF to request refills4Assist with clinic flow for in house providers and Tele-Psych visitsInterested in hearing more?
